Problem:
Cancer specialists at Tata Memorial had no structured way to collect, organize, and centralize patient data across the different cancer registries their research and clinical work depended on. Data was fragmented across manual notes and disparate Excel sheets, with no consistent way to track a single patient's data over the full course of their treatment (longitudinally) or to capture the depth of structured clinical detail each registry actually needed at any given visit (vertically). For procedure-specific registries like Vacuum-Assisted Breast Biopsy (VABB), that gap meant valuable clinical and research data was being lost or left unusably inconsistent.
Solution:
Latent built Formly as a web-based disease registry platform for doctors across Tata Memorial's hospitals:
- Multiple disease registries on one platform — rather than a single form, Formly supported a collection of registries, each structured around the specific data a given cancer type or procedure required, including registries for procedures like VABB.
- Longitudinal data capture — the same patient's data connected and trackable across multiple visits and over the course of treatment, not re-entered from scratch each time.
- Vertical data capture — deep, structured clinical fields captured at each individual data point, built to match the specificity each registry's research and clinical use actually required.
- Mobile-responsive, on-the-go data entry — a fully mobile-responsive interface so specialists could capture data during rounds or clinic, not just at a desktop.
- Intelligent auto-fill — auto-fill functionality to speed up data entry and reduce inconsistency across a high volume of structured fields.
Impact:
Formly turned data that was previously scattered across manual notes and spreadsheets into a centralized, structured system built around how oncology research and clinical data actually needs to be captured — longitudinally per patient and vertically per data point. That gave Tata Memorial's specialists comprehensive, actionable data for both individual patient care and broader clinical research across registries like VABB, and removed the data-quality and accessibility problems that came with an ad hoc, spreadsheet-based process.
